Understanding Perimenopause & Menopause
Perimenopause
Perimenopause refers to the transitional phase preceding menopause, during which ovarian hormone production begins to fluctuate and decline. This phase can last several years and begins, on average, in a woman's mid-to-late 40s — though it can start earlier. Menstrual irregularity, vasomotor symptoms, sleep changes, and mood disturbances are common during this time.
Menopause
Menopause is defined as the cessation of menstruation for 12 consecutive months in the absence of other pathological causes. The average age of natural menopause is approximately 51 years. Surgical menopause (following oophorectomy) and premature ovarian insufficiency are additional scenarios requiring specialist management.
Common Menopause Symptoms
Cardiovascular risk changes
Sleep disturbances and insomnia
Joint and muscle aches
Skin and hair changes
Reduced libido
Hot flashes and night sweats (vasomotor symptoms)
Mood changes, anxiety, and low mood
Cognitive changes — concentration and memory difficulties
Reduced bone density (increased osteoporosis risk)
Vaginal dryness, discomfort, and urinary symptoms (genitourinary syndrome of menopause)
Menopause Management Approaches
Treatment is always individualised, based on symptom severity, personal health history, risk factors, and patient preference.
🔹 Hormone Replacement Therapy (HRT)
The most effective treatment for vasomotor symptoms; evidence also supports bone and cardiovascular benefits. Prescribing follows current RCOG, NICE, and FIGO guidance.
🔹 Non-hormonal treatments
For women where HRT is not appropriate or preferred, evidence-based non-hormonal options are available for vasomotor and other symptoms.
🔹 Genitourinary management
Targeted local oestrogen therapy specifically for vaginal and urinary symptoms.
🔹 Bone health assessment
DEXA scanning guidance, calcium and vitamin D optimisation, and proactive lifestyle recommendations.
🔹 Lifestyle counseling
Guidance on nutrition, physical activity, sleep hygiene, and overall cardiovascular risk reduction.
🔹 Mental health support
Specialised recognition and management of menopause-related mood disturbances and anxiety.
